Nasal tip surgery: harmony with the whole face matters more than height alone

Nasal tip surgery should harmonize the tip’s height, length and direction and the shape of the nostrils with the entire face, rather than simply raising the tip. Because the tip affects the nose’s overall appearance and balance, planning should consider its shape and direction and its relationship to the columella and nostrils, not just nasal height.

Correction may be considered, depending on the individual, not only for a very low or blunt tip but also for a drooping or asymmetric one. The key is to achieve a natural shape and balance based on the nose’s overall structure and facial proportions, rather than simply raising the tip.

The tip’s shape depends on anatomical factors including skin thickness, cartilage shape, nasal length, columellar structure and nostril shape. Options may therefore include reshaping the tip cartilage or, when needed, using the patient’s own cartilage to strengthen tip support. Columellar correction may also be combined with these techniques.

When only the tip is being corrected, a closed approach using incisions inside the nose may be considered, depending on the extent of surgery and the nasal structure. Because this method does not directly incise the external skin, it can reduce concerns about visible incision scars. However, a closed approach is not suitable for every nasal tip operation.

Rather than applying the same technique to everyone, the appropriate method should be chosen after assessing the individual’s nasal structure, skin condition, required correction and history of rhinoplasty. The best approach can differ according to tip skin thickness, cartilage shape, nasal structure and the degree of correction. Before deciding on surgery, an accurate assessment and a thorough discussion with the medical team about the necessary correction, technique and expected changes are therefore needed.

Although nasal tip surgery changes a small area, it can influence the balance of the entire nose and the impression of the face. Planning should therefore consider individual anatomy and facial proportions comprehensively, rather than focusing only on improving height and shape.
